Anyone living in the practice area is welcome to register. A map of the catchment area is available on request. You will be asked to: Complete a registration form (FP1) You will be given an appointment with one of the healthcare assistants for a health check – please ask for a bottle to bring a urine sample with you. You may be invited to make an appointment with one of the doctors or practice pharmacist if you are on regular medication.
The practice may feel that it has justifiable reason to refuse an application for registration under the guidelines laid down in the GMS2 contract. In these circumstances written explanation of the reasons for refusal will be forwarded to the person concerned.
